The role of sipping in the nutritional support of geriatric patients

Introduction: Malnutrition is a significant problem in old age. Malnutrition is present in 30 - 70 % of hospitalized elderly and its consequences are severe. Systematic screening and early treatment are essential to ensure and maintain good health in the elderly and are an integral part of overall geriatric care. Nutritional support with sipping - oral nutriti- onal supplements (ONS) is one of the basic approaches in the management of malnutriti- on in geriatric patients. Aim of the thesis: The main aim of this thesis was to evaluate the intake of sipping in geri- atric patients and to monitor possible pitfalls in its use. To answer the main objective, four research questions were set to provide the following answers: 1. What proportion of the total energy intake (CEP) of a patient is from diet and from sipping, and how does gender and age affect energy intake from sipping ? 2. What proportion does dietary and sipping protein make up of the patient's prescribed intake? 3. Will the diet, including sipping, cover the patient's energy and protein needs? 4. Will patients drink their prescribed dose of sipping, and will certain factors such as education by the nutritional therapist and ability to eat affect the patient's in- take of sipping? The nurse´s role in the care of clients´ nutrition in retirement homes

The bachelor thesis is title "The nurse´s role in the care of clients´ nutrition in retirement homes." It contains four objectives. The first objective is to find out how general nurses evaluate the nutrition of nursing home clients. The second objective focuses on nurses' knowledge of the subject. The third objective determines whether the general nurse adheres to standards in the nutritional care of the elderly. The fourth objective is to find out what are the specifics of nutrition of the elderly. The bachelor thesis is divided into theoretical and practical part. The theoretical part describes nutritional specifics of the elderly, organ changes in old age, nutritional components and drinking regime. It also focuses on nutritional disorders in the elderly, malnutrition, obesity and dehydration. The last part is nutritional support for the elderly. The role of the nurse in nutritional care of the elderly, nutritional assessment, enteral and parenteral nutrition are described. The practical part includes results obtained from a research investigation. This part was conducted by qualitative investigation, semi-structured interview method and observation. The interview includes twenty-two open-ended questions and four questions involving identification of probands. For the observation, a table was previously created which includes thirteen aspects. The research sample consisted of ten general nurses working in a nursing home. From the results of the qualitative survey it is evident that most nurses use nutritional screening and physical examination in the form of measuring the client's weight to assess the nutritional status. It was also found that the level of knowledge of nurses is sufficient. The nurses are knowledgeable in the subject matter. They are knowledgeable about malnutrition and dehydration. It is also evident from the interviews that they are knowledgeable about the standard that deals with nutrition care for the elderly. The observation revealed the mistakes that nurses make when serving meals to the elderly. The main errors include not using clothing protection and lack of communication with the senior. Some of the more unimportant specifics of nutrition include a balanced and varied diet. It is important that the diet is adequately represented by all nutritional components.
Quality of life of selected groups of clients with Crohn's disease

My dissertation work called "The quality of life within selected groups of clients with the Crohn's disease" is analysing the subjective evaluation of the clients' life situation, that describes their day-to-day struggle with this chronic disease. This work is divided into two parts, theoretical and empirical. The objective of the former is to present the current findings about the Crohn's disease, to describe the term "quality of life", defines its various meanings and history and specifies the assessment methods in the clinical practice. In the latter, there are set objectives and working hypotheses for own evaluation. The target of the empirical part is to obtain real life data through questioning methods, partly based on the standardised WHQL-BREF methodology. The obtained data will be subjected to primary analysis in order to create an evaluation result for a selected group of clients. In the last part, this dataset will be compared with the previously measured values on quality of life of clients with Crohn's disease.
The Nutrition Day at the standard and intensive care beds in the teaching hospital

The Bachelor thesis consists of two parts - the theoretical and the practical one. The theoretical part is divided into several chapters; the first one is focused on the main information about malnutrition such as its definition, prevalence, classification, causes, impact and the methods of diagnosing. The following chapter describes the algorithm for nutritional care; the rest of the chapters deal with the artificial nutrition, where the indications, contraindications, complications and other essential information related to enteral and parenteral nutrition are described. The practical part is dedicated to the worldwide programme "Nutrition day" and also to the evaluation of the data given by patients of the 4th internal clinic VFN in Prague during the programme. The research part was carried out on grounds of quantitative survey through a standardized questionnaire. There are two aims to the study; the first one was to find out the food intake of all patients of the 4th internal clinic. This goal was not accomplished, because not every patient placed in the standard ward was willing to complete the questionnaire. However, the study showed that only 29 % out of 62 patients who completed the questionnaire ate the whole portion of meal. The second aim was to find out if the daily calorie intake...
Nursing Issues in Patients with PEG

Bachelor thesis were written on theoretic-empirical bases. The theoretical part is dedicated to general introduction into anatomy and physiology of stomach. The following chapter is about nutrition, the explanation of the terms malnutrioton, nutritional support, enteral nutrition as well as its indications, contraindications, complications, types and forms of artificial nutrition included. The core chapter is about the implementation and treatement of PEG, the psychosocial aspects and specifice of home care included. The aim of the empiric part is to find out to what extend is the issue of PEG familiar to nurses, what are the possibilities in treatement of the patients with PEG, how the patients react to the PEG treatement and nutrition at home. A quantitative method of annonymous questionnaires was used to effectuate this survey. The tested sample formed 111 respondents/nurses and 22 respondents/patients with PEG in home care. The results of the research show that 99% of nurses are familiar with the PEG and 86% are convinced about having enough information how to treat the patient wih PEG. 66% of respondents/nurses are not familiar with any complications during PEG in hospital. Nutritional support in patients with neurological disease

This diploma thesis deals with the topic of nutritional support in patients with neurological diseases (stroke and Critical-Illness-Polyneuropathy). The aim of this thesis is to find out, how implemented unified system of nutritional support affects the well-being of patients at neuro-rehabilitation clinic Asklepios Schlossberg Klinik in Bad König. There were observed 55 patients (30 men and 25 women) for 8 weeks. The theoretical part of this thesis describes basic components of nutrition, energy expenditure and needs, selected neurological diseases, dysphagia, malnutrition. The emphasis is put on nutrition in intensive neurological care. The practical part of the thesis analyses data obtained by the observation. There are observed changes of body weight, BMI and laboratory values of total protein, albumin and glycaemia in serum. These data are divided by sex, age, type of disease and initial BMI values. The observed patients had mainly overweight and obesity, which have lost a part of their excess weight due to catabolism. At the beginning of the observation 13 patient had normal BMI value, this number of patients rose to 24 after eight weeks. This diploma thesis deals with the topic of nutritional support in patients with neurological diseases (stroke, craniocerebral trauma, critical-illness-polyneuropathy. The aim of this thesis is to find out, how implemented unified system of nutritional support affects the well-being of patients at neuro-rehabilitation clinic Asklepios Schlossberg Klinikum in Bad König. Sufficient and quality nutrition is an important support for mastering serious oncological treatment and debilitating illness. Educating the public about the risk of oncological disease related malnutrition and the importance of a balanced diet, incidence of new malignanccies increases every year. 94 000 people as the newly diagnosis were in 2015. Nearly 542 000 people lived with cancer diagnosis in 2015. Not only among patient it is widespread idea that malnutrition is something that can not concern us because we are doing good. Many patients do not have insight into their nutritional status. They do not think they are at risk of malnutrition. But if someone gets cancer, sooner or later, the symptoms of the disease will appear, and along with demanding treatment that brings a lot of side effects, and worsing of oral intake, and weight loss is coming. The theoretical part describes the causes of the cancer disease, its influence on the organism, the most common types of anticancer treatment together with the side effects and the possibilities of their management. Nutritional status of patients with colorectal cancer before the diagnosis and during the therapy

This diploma thesis focuses on the nutritional status of patients with colorectal cancer. In the theoretical part I describe the incidence, risk factors of colorectal cancer, clinical manifestations, diagnostics, individual types of anticancer therapies, their side effects and dietary recommendations for individual gastrointestinal side effects of anticancer therapy. In the theoretical part I also focus on malnutrition and nutritional support in the treatment of colorectal cancer. Also important is the prevention of colorectal cancer, which is described in the last section. The aim of the research is to compare the eating habits and lifestyle of patients with colorectal cancer before the diagnosis and during the therapy. The second objective is to compare the weight of the patients before the diagnosis and therapy and to find out the most common causes of weight loss in anticancer therapy.
